Agreement
I authorize Tucson Yoga to deduct $45 per month from the credit/debit
account I specify below, which will authorize me to attend unlimited
yoga classes. The monthly debit will occur on today's date each month.
I agree to commit to two full months of membership, after which I
may cancel this agreement at any time by calling Tucson Yoga at 1-877-TUC-YOGA.
(Call must be received at least three days before the next scheduled
debit.) I understand that my card will be debited or charged indefinitely until
I call to cancel. I understand that each month is paid in full in
advance, and that there will be no refund for a full or partial month after
a deduction is made.
